If your child is in my class, parents should have received a syllabus and an information sheet.  The syllabus is yours to keep.  The information sheet can be filled out by the parent and returned by your child or it can be completed online by the parent.  If the form below is completed within the time stated on the syllabus that your child received, I will accept it as on time and your child will receive credit.  Thank you!
